Returning your documents: please email both completed documents to pump@edmontondiabetes.ca before your appointment. If your appointment is in person, you can print them and bring them in with you.

Cancelling or rescheduling

No-shows or cancellations with less than 24 hours' notice will incur a $50 fee. To cancel or reschedule, please call or email us.
