How to get seen
Diabetes care at our clinic requires a referral from your family doctor or nurse practitioner. Referring is simple — a form or a letter works.
- Talk to your doctor or nurse practitionerAsk them to refer you to Edmonton Diabetes for diabetes care.
- They send the referralBy fax to (780) 409-5670 or by email. Our referral form is helpful, but a letter is fine.
- We call you to bookOnce we receive your referral, we contact you to schedule your first visit.
- Your first visitA full review of your diabetes and a plan you understand, with follow-up as you need it.
No family doctor? A nurse practitioner or walk-in clinic doctor can refer you.
Have Type 1 diabetes and want to start or switch an insulin pump? You may not need a referral — certain patients can book themselves through our self-referred pump program.

Care that connects
Diabetes care FAQ
Do I need a referral?
Yes. Ask your family doctor or nurse practitioner to refer you. They can fax a referral to (780) 409-5670 or email it — our referral form or a letter both work. The exception: certain patients with Type 1 diabetes can book an insulin pump consult themselves through our self-referred pump program.
Is it covered?
Yes. Consultations and follow-up visits with our physicians are covered by Alberta Health.
What should I bring?
Your CGM and reader (or the phone you use with it), or your glucometer and logbook — plus your actual medications, not just a list. You don't need to bring your Alberta Health card.
What if I don't have a family doctor?
A nurse practitioner or walk-in clinic doctor can refer you.
Will my family doctor be kept in the loop?
Yes. We send consult letters back to the doctor who referred you and upload them to Netcare.
