Referring a patient
We're accepting new referrals for all services. Send our referral form or a simple letter — by fax or email. We want referring to be easy.
Services and typical waits
All consultations and follow-ups below are covered by Alberta Health.
Wound care
Diabetic foot ulcers and any open wound below the knee, with or without diabetes. A recognized AHS High Risk Foot Clinic. Program details →
Diabetes
Type 2, type 1 and all other types of diabetes, with specialist physicians and certified diabetes educators.
Insulin pumps
Starting or switching an insulin pump for people with type 1 diabetes, with our certified pump trainers.
Endocrinology
Thyroid and other hormone conditions.
What to include
Any referral works
Our referral form is helpful, but a letter is fine too. Include the patient's contact information and the reason for referral.
- DiabetesHelpful if available: recent A1c, urine ACR, lipid panel and creatinine.
- Wound careNothing specific needed.
- EndocrinologyAny relevant recent labs or imaging.
After you refer
- We contact your patientOur team calls to book their first visit.
- We keep you informedConsult letters are faxed back to you and uploaded to Netcare.
- We coordinate careWhen needed, we arrange referrals to vascular surgery, orthopedics, plastics, infectious diseases and other specialists.
Send these patients to emergency instead
We're not an urgent care clinic. Patients with any of the following should go directly to the nearest emergency department:
- Acute limb ischemia
- Acute joint or skin infection
- Suspected diabetic ketoacidosis
- Acute onset of pain in a previously insensate foot
After hours, direct patients with these concerns to the nearest emergency department.
Foot care your patients can book themselves
Medical pedicures, Onyfix ingrown toenail correction, ToeFX fungal nail therapy and compression stockings (AADL vendor) — open to anyone, with or without diabetes.
